CMEs:
18.0 Category 1 credits toward the AMA Physicians
Recognition Award will be given for full participation.
Registration Fee of
$400 includes participation in all sessions, a
handout of the speakers' presentations, lunches
and coffee breaks. The fee is valid until January
16, 2010. After that (including on-site registration)
the fee is $440.
Accommodation:
Rooms are available at the Conference Center for
$90 per night for a single and $110 for a double
room. Prices are for B/B. A nonrefundable deposit
of $110 must accompany reservations.
Cancellation Policy:
Cancellations received by December 28, 2009 are
not subject to a cancellation fee. Cancellations
received between December 28, 2009 and January
17, 2010 are subject to a $60 service charge.
No refund for cancellations received after January
17, 2010.
Conflict of Interest
Statement:
The "Conflict of Interest Disclosure Policy" of
Albert Einstein College of Medicine requires that
faculty participating in any CME activity disclose
to the audience any relationship(s) with a pharmaceutical
or equipment company. Any presenter whose disclosed
relationships prove to create a conflict of interest
with regard to their contribution to the activity
or who refuses to provide all their conflict of
interest information, will not be permitted to
present.
The Albert Einstein College of Medicine also requires
that faculty participating in any CME activity
disclose to the audience when discussing unlabeled
or investigational use of any commercial product
or device not yet approved for use in the United
States.
